What Is the Medical Transportation Initiative?
The Medical Transportation Initiative is a targeted program designed to provide safe, reliable, and affordable transportation for Medicaid and Medicare recipients in Vermont. By partnering with local organizations and community-based transport services, we are ensuring that vulnerable residents in rural and underserved areas can access essential healthcare services without transportation barriers.

Key Goals:
  • Provide transportation for over 500 Medicaid and Medicare recipients to medical appointments, pharmacies, and essential services.
  • Offer 10,000 trips annually to reduce missed appointments and unnecessary emergency room visits.
  • Decrease vehicle miles traveled by 150,000 miles, reducing both costs and environmental impact.

Why This Matters:
For many low-income residents, transportation to medical appointments is not just a challenge; it is a significant barrier to receiving necessary care. Without reliable access, individuals often face poorer health outcomes, higher healthcare costs, and an increased risk of hospitalization.

The Urgent Need for Your Support

Transforming Dementia Care for Vermont's Older Adults

At Lyght Bulb Moment Foundation Inc., we are dedicated to enhancing the health and well-being of older adults and underserved communities in Vermont.
Through the Loretto Legacy Project, we aim to provide compassionate, person-centered care that addresses critical gaps in dementia care, mental health, and primary care.

What is the Loretto Legacy Project?
The Loretto Legacy Project is a comprehensive initiative designed to meet the urgent need for high-quality dementia care and support services in rural Vermont. Recent closures of long-term care facilities have created a severe shortage of residential care options, leaving many families without the resources they need for their loved ones. According to
Vermont Public (November 30, 2023), this shortage is creating a crisis in care that demands innovative, community-based solutions.

Our project addresses these challenges by:
  • Expanding dementia care and mental health services through partnerships with three Genesis Healthcare facilities in Vermont.
  • Collaborating with community organizations like the Chronic Care Initiative, Support and Services at Home (SASH), and the Vermont Office of Area Aging to provide integrated care.
  • Reinvesting revenue generated from these partnerships—potentially upwards of $500,000 annually—to grow our programs and reach more individuals in need.
Your support is critical to expanding this work and ensuring no one is left without care.

Why Now?
The urgency of the Loretto Legacy Project has never been greater. As more long-term care facilities close, the need for community-based care models is rapidly increasing. We are prepared to step in and fill this gap, but we need your help to scale our efforts and meet the growing demands.

We are seeking $18,808,000 in funding to:
  • Hire and train dementia care specialists to work directly with nursing home residents and staff.
  • Develop digital coaching and support platforms to provide on-demand training and reduce caregiver burnout.
  • Facilitate ongoing workshops and support groups for families, caregivers, and healthcare providers.

What is the Healthy Horizons Project?
The Healthy Horizons Project is inspired by the PACE (Program of All-Inclusive Care for the Elderly) model, which emphasizes coordinated, team-based care to meet a patient’s full range of needs—medical, social, emotional, and functional.

However, our project expands beyond the traditional PACE model to include:
  • Telemedicine and Mobile Health Units: Reaching patients directly in their homes and communities, providing comprehensive care without the need for long-distance travel.
  • Community-Based Partnerships: Collaborating with local organizations like the Chronic Care Initiative, Support and Services at Home (SASH), and the Vermont Office of Area Aging to create a seamless continuum of care.

By combining telemedicine with in-person services, we promote independent living while reducing reliance on emergency room visits and high-risk medications. The Dementia Care Initiative is a pilot project designed to significantly enhance the quality of dementia care in three to five nursing homes in Vermont. By integrating advanced digital coaching platforms and providing continuous support for nursing home staff, we aim to improve patient outcomes, increase staff satisfaction, and elevate overall care quality.

Key Goals and Objectives:
  • Integrate Dementia Care Specialists: Hire and train specialized professionals to support nursing home teams.
  • Implement Digital Coaching and Training: Develop and provide a secure virtual platform equipped with advanced analytics for continuous staff education and support.
  • Reduce Burnout and Improve Retention: Enhance staff retention and satisfaction by reducing burnout and promoting skill development.
  • Improve Patient Outcomes: Focus on reducing adverse events and effectively managing dementia-related behaviors to improve the quality of life for residents.
